How Stroke Changes the Brain’s Ability to Understand Speech

A stroke can change many aspects of daily life, but one of the most frustrating and least visible effects for many survivors is difficulty understanding spoken language. New neuroscience research is now offering clearer answers about what exactly happens in the brain after a stroke that affects speech comprehension. Instead of focusing on hearing loss or slow thinking, the study points to something more subtle: changes in how the brain processes and holds on to speech sounds.

This research, led by Laura Gwilliams from Stanford University and Maaike Vandermosten from KU Leuven, takes a close look at the brain activity of people who developed language difficulties after a stroke. Their findings were published in the Journal of Neuroscience in 2025 and provide important insights into aphasia, a common language disorder caused by stroke.


Understanding Speech Problems After Stroke

Many people assume that when stroke survivors struggle to understand speech, it means they either hear sounds poorly or process them too slowly. However, this new research challenges that idea.

The researchers studied 39 people who had experienced a stroke and were living with speech comprehension difficulties, and compared them with 24 healthy adults of similar age. All participants listened to a spoken story while their brain activity was recorded. This setup allowed scientists to observe how the brain naturally responds to speech, rather than relying on artificial lab tasks.

What they discovered was surprising: stroke survivors were not slower at detecting speech sounds. Their brains picked up phonemes—the basic units of speech—at roughly the same speed as healthy participants. The real difference lay elsewhere.


Weaker Brain Responses, Not Slower Ones

Although the timing of speech sound processing was similar between the two groups, the strength of the brain’s response was much weaker in people who had experienced a stroke. In simple terms, their brains heard the sounds, but the signals were less robust and less sustained.

This distinction is important. It suggests that stroke-related language disorders are not about failing to hear sounds, but about struggling to integrate those sounds into meaningful words and sentences. The brain detects the building blocks of speech, but does not process them deeply enough to support clear understanding.

The study shows that stroke survivors can hear speech sounds just as well as anyone else, but the brain has difficulty holding and combining those sounds long enough to make sense of them.


What Happens When Speech Is Unclear

The researchers also explored how the brain reacts when speech becomes uncertain or difficult to interpret. For example, when words are muffled, ambiguous, or spoken in a noisy environment, the brain usually works harder to figure them out.

In healthy participants, brain activity showed that speech sound features were processed for a longer period when there was uncertainty. This extended processing gives the brain extra time to resolve ambiguity and identify the correct word.

In contrast, people who had experienced a stroke did not show this prolonged processing. Their brains stopped analyzing speech sounds sooner, even when the information was unclear. This may explain why stroke survivors often find it especially hard to understand speech in real-world situations like crowded rooms or fast conversations.

In other words, after a stroke, the brain may give up too early when speech is difficult to decode.


Key Brain Mechanisms Revealed

This research highlights specific brain activity patterns that appear to be crucial for understanding spoken language. The findings suggest that successful speech comprehension relies not just on detecting sounds, but on maintaining and strengthening neural representations of those sounds over time.

The study’s first author, Jill Kries, emphasized the value of using natural listening tasks, such as listening to a story. Traditional language assessments often involve hours of repetitive behavioral tests, which can be exhausting for patients. By contrast, simply recording brain activity while someone listens to speech could offer a faster and more accurate diagnostic tool for language disorders.

This approach could significantly improve how clinicians assess aphasia and similar conditions in the future.


What Is Aphasia and Why It Matters

Aphasia is a language disorder that affects a person’s ability to understand or produce speech. It most commonly occurs after a stroke that damages language-related areas of the brain, usually in the left hemisphere.

People with aphasia may:

  • Struggle to understand spoken language
  • Have trouble finding the right words
  • Speak fluently but with incorrect or nonsensical words
  • Understand individual sounds but not whole sentences

Importantly, aphasia does not affect intelligence. The person knows what they want to say or understand, but the brain’s language network is disrupted.

The new study adds to this understanding by showing that aphasia may stem from reduced neural strength and persistence, rather than delayed processing.


Why Timing and Strength Both Matter

Speech comprehension is a fast and complex process. The brain must quickly detect sounds, categorize them into phonemes, combine them into words, and interpret meaning—all in fractions of a second.

This research shows that initial sound detection remains intact after stroke, but later stages of processing are weakened. The brain does not maintain strong enough activity to support comprehension, especially when speech is challenging.

This insight shifts the focus of stroke-related language research. Instead of asking whether speech is processed too slowly, scientists are now asking whether it is processed deeply and long enough.


Implications for Recovery and Therapy

Understanding these brain changes could influence how speech therapy is designed. If the problem lies in weak and short-lived processing, therapies might focus on:

  • Strengthening neural responses to speech sounds
  • Training patients to tolerate ambiguity longer
  • Improving comprehension in noisy or complex listening environments
